Terms of Service

Precision Credentialing Solutions LLC — Miami, Florida

1. Agreement to Terms

By engaging the services of Precision Credentialing Solutions LLC ("Company," "we," "us," or "our"), you ("Client") agree to be bound by these Terms of Service. These terms govern the credentialing, payer enrollment, contracting, and related administrative services provided by the Company.

A separate Service Agreement will be executed prior to the commencement of any work. These Terms of Service are incorporated by reference into that Service Agreement.

2. Services Provided

Precision Credentialing Solutions LLC provides the following services:

  • Provider credentialing setup and management
  • Medicare and Medicaid enrollment
  • Commercial payer enrollment and contracting
  • CAQH ProView profile setup and maintenance
  • Proactive compliance and expiration monitoring
  • Credentialing recovery and cleanup
  • Ongoing monthly maintenance plans
  • Real-time status reporting

The specific scope of services will be defined in the Service Agreement signed by both parties.

3. Client Responsibilities

The Client agrees to:

  • Provide accurate, complete, and current information and documentation as requested
  • Respond to requests for additional information in a timely manner
  • Notify the Company immediately of any changes to professional licenses, certifications, addresses, or other relevant information
  • Ensure all documents provided are authentic and unaltered
  • Authorize the Company to act as a liaison with insurance payers and networks on their behalf
  • Maintain active professional licenses and certifications throughout the engagement

4. Payment Terms

One-time packages: A deposit is required to initiate services. The remaining balance is due upon completion of the agreed scope of work.

Monthly maintenance plans: Billed monthly on a recurring basis. Plans may be cancelled with thirty (30) days written notice.

Accepted payment methods: ACH transfer, credit card, and check.

Late payments: Invoices unpaid after 30 days may be subject to a late fee of 1.5% per month on the outstanding balance.

5. Timeline and Payer Processing

The Company will use commercially reasonable efforts to process credentialing applications in a timely manner. However, the Client acknowledges that:

  • Credentialing timelines are largely determined by insurance payers and are outside the Company's control
  • Medicare and Medicaid enrollments typically take 60–90 days
  • Commercial payer enrollments typically take 60–180 days
  • Delays caused by incomplete documentation, payer backlogs, or Client unresponsiveness are not the responsibility of the Company

The Company does not guarantee enrollment with any specific payer or network.

6. Confidentiality

Both parties agree to maintain the confidentiality of all proprietary and sensitive information shared during the engagement. The Company will not disclose Client information to third parties except as necessary to perform the contracted services or as required by law.

7. Limitation of Liability

Precision Credentialing Solutions LLC is a credentialing consulting firm. We do not provide legal, medical, billing, or financial advice. Our services are administrative in nature.

The Company's total liability for any claim arising out of or related to these Terms shall not exceed the total fees paid by the Client in the three (3) months preceding the claim. The Company shall not be liable for any indirect, incidental, special, consequential, or punitive damages.

8. Termination

Either party may terminate the service engagement with thirty (30) days written notice. Upon termination:

  • The Client remains responsible for fees incurred through the termination date
  • The Company will provide the Client with all relevant account credentials and status reports
  • Any deposits for incomplete work are non-refundable unless otherwise agreed in writing

9. Governing Law

These Terms of Service shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be subject to the exclusive jurisdiction of the courts located in Miami-Dade County, Florida.

10. Changes to Terms

We reserve the right to modify these Terms at any time. Changes will be posted on our website with an updated date. Continued use of our services after changes are posted constitutes acceptance of the modified Terms.
